Incident Overview

Date: Wednesday 23 June 2004
Aircraft Type: Dornier 228-201
Owner/operator: Islandsflug
Registration Number: TF-ELH
Location: Siglufj”rdur Airport (SIJ) – ÿ Iceland
Phase of Flight: Landing
Status: Destroyed, written off
Casualties: Fatalities: 0 / Occupants: 2
Component Affected: Landing gearLanding gear
Investigating Agency: RNFRNF
Category: Accident
During a training flight, an airplane sustained significant damage after landing at Siglufj”rdur Airport (SIJ) in Iceland. The aircraft was transported by boat to Reykjavik. It was re-registered as TF-ADD for Landsflug but was deemed beyond repair due to damage to the landing gear, failure to utilize checklists, disconnection of the ground proximity warning system, and pilot uncertainty regarding the approach. The flight path, flying towards the mountains rather than the fjord, constrained the aircraft’s maneuverability.During a training flight, an airplane sustained significant damage after landing at Siglufj”rdur Airport (SIJ) in Iceland. Description

The airplane was substantially damaged after it made a gear-up landing at Siglufj”rdur Airport (SIJ) during a training flight. The aircraft was transported to Reykjavik by boat. In November 2004 it was re-registered TF-ADD for Landsflug, but was ultimately assessed as damaged beyond repair. The points considered to be the most likely causes of the accident are: ? The landing gear was not lowered. ? Checklists were not used after the first landing. ? The ground proximity warning system was disconnected by the crew. ? The pilot took control of the aircraft unsure of how the next approach should be carried out. ? Since the traffic pattern was flown to the east (i.e., towards the mountains rather than over the fjord), there was little room due to the confined space.

Primary Cause

Failure to properly lower the landing gear, inadequate use of checklists, disconnection of the ground proximity warning system, pilot uncertainty regarding the approach, and the constrained flight path.Failure to properly lower the landing gear, inadequate use of checklists, disconnection of the ground proximity warning system, pilot uncertainty regarding the approach, and the constrained flight path.
